Payment Policy

Payment is due at the time of service, for evaluations, consultations, office visits, and for any medical services deemed necessary for your care. Personal or business checks or cash are acceptable methods of payment. As a courtesy to our patients, we also accept your Visa or MasterCard credit and debit cards.

Because we hold your confidential information in the highest regard, Dr. Hillenbrand is not under contract with any insurance company or health plan organization. The healthcare contract exists between the doctor and the patient. While a patient may have health insurance, Dr. Hillenbrand will be out of network for that insurance. Before making an appointment, please contact your carrier to determine your rate of reimbursement for services rendered by out-of-network physicians. You must also determine whether preauthorization is required before seeing Dr. Hillenbrand.

After each appointment, you will receive a fully-coded claim form.  You may then submit this form – with a copy of your insurance card attached – for reimbursement to your insurance company. The instructions for submitting the claim are found on the back of your claim form. No additional forms need completion in order to implement the entire billing process. The staff is available to help you with any questions that may arise.
